RD Physical Therapy and Wellness, LLC does not participate with any health insurance companies at this time. The increasing paperwork and decreasing reimbursements from insurance makes it very difficult to manage our practice and continue to provide the type of care that we give to our patients.

Payment for services will be due at the time of service. Please be prepared to pay when you arrive at the office.  We will accept cash, checks, and credit cards. We will be unable to honor any future appointments until the fee is paid.

Fee Structure:

• PT evaluation: $75
• PT evaluation and treatment: $105.00
• Routine Treatment: $90.00

We accept credit cards.

At the time of you visit, we will provide you a form to submit to your insurance provider for reimbursement.

You should contact your insurance company prior to your appointment to determine if you have any out-of-network coverage and if there is any deductible to be met prior to receiving any reimbursement from you insurance provider.

Also contact your insurance companies to find out if you need a script, referral, or prior authorization. Our office will gladly assist in faxing prior authorization requests, if needed.
